Dr. Rod W. Gore is offering a new in Office Comprehensive Dental Plan. This plan is for those patients that may not have Dental Benefits. We have created a unique and innovative Plan designed sepcifically to provide affordable access to quality family dental care. Our GOREgeous Membership Plan offers patients far more benefits than traditional insurance plans as described below:

Annual Fee
- Adults – $600
- Children – $400 (under 18)
- Couple – $1100
Program Guidelines and Limitations
- You will not receive a membership card – your plan’s effective date will be on file with our financial coordinator and will be effective for 12 months, beginning on the date of purchase. Notification of renewal will be mailed and the plan must be renewed prior to the expiration date.

- All payments are DUE AT THE TIME OF SERVICE
